Open Positions
Registered Nurse (RN)
Full-Time / Part-TimeProvide skilled nursing care including assessments, wound care, medication management, and patient education. Pediatric experience a plus.
- Active Illinois RN license
- Minimum 1 year clinical experience
- Valid driver's license and reliable transportation
- Strong communication and documentation skills
Licensed Practical Nurse (LPN)
Full-Time / Part-TimeDeliver hands-on nursing care under RN supervision, including wound care, medication administration, and vital sign monitoring.
- Active Illinois LPN license
- Home health experience preferred
- Valid driver's license and reliable transportation
- Compassionate patient-centered approach
Pediatric Home Health Nurse (RN/LPN)
Full-Time / Part-TimeSpecialized nursing care for pediatric patients, including feeding tube management, tracheostomy care, and complex medication regimens.
- Active Illinois RN or LPN license
- Pediatric nursing experience required
- Experience with trach and/or feeding tube care preferred
- Ability to work closely with families and caregivers
Certified Nursing Aide (CNA)
Full-Time / Part-Time / PRNAssist patients with activities of daily living, personal care, and mobility support under the supervision of a licensed nurse.
- Active Illinois CNA certification
- Home health or long-term care experience preferred
- Valid driver's license and reliable transportation
- Reliable, punctual, and compassionate
